A 24-year-old woman was accidentally killed on Tuesday when she walked into the spinning propeller of a Twin Otter on the ramp at the Ohio skydiving business where she worked.

Sarah Rhoads, a well-liked manager for Start Skydiving at Middletown Regional Airport, was pronounced dead at a local hospital after suffering severe head injuries, authorities said.

Although reports said Rhoads was trained in how to safely work the ramp, the Twin Otter was a brand new aircraft for the company, which also operates a single-engine Cessna 206 and Caravan. She had worked at Start Skydiving for the last three years.

The FAA said it will investigate the accident.
