At about 1800 Eastern time, the airplane was substantially damaged during a forced landing following a partial loss of engine power after takeoff from a private airstrip. The private pilot was not injured. Visual conditions prevailed. The pilot later reported the airplane sat for approximately a month prior to the accident flight, with its wing fuel tanks about one-quarter full. His preflight inspection included sumping the fuel tanks, and he thought he had removed all water and contamination from the fuel tanks.

November 14, 2010, Tucson, Ariz., Cessna 150J

The airplane was substantially damaged at about 1215 Mountain time when it impacted terrain while on final approach. The solo student pilot was killed. Visual conditions prevailed for the local supervised solo flight. After completing five or six uneventful takeoffs and landings, the CFI exited the airplane to watch the student conduct a solo flight in the airport traffic pattern.

September 10, 2010, Fayetteville, W.V., Cessna 150

The airplane incurred substantial damage during a forced landing at about 1315 Eastern time following a partial loss of engine power. The flight instructor and private pilot received serious injuries. Visual conditions prevailed. During initial climb the tachometer indicated 1900 and the airplane would not gain altitude. The flight instructor elected to land in a field, but the airplane collided with trees before impacting the ground.

The airplane sustained substantial damage at about 0650 Pacific time after colliding with terrain and power lines. The student pilot was killed and his sole passenger received serious injuries. Visual conditions prevailed for the local flight.

August 12, 2009, Sussex, N.J., Cessna 150

The airplane was substantially damaged at 0830 Eastern time after losing lift during takeoff. The private pilot and a passenger received serious injuries. Visual conditions prevailed at the time.According to the pilot, during takeoff, the airplane would not gain altitude and started clipping the tops of trees off the end of the runway. The pilot pulled back on the yoke and stalled the airplane. The airplane collided with an embankment and came to rest in a creek. The airplane impacted the creek in a near-vertical nose-down attitude.

June 16, 2009, Terrell, Texas, Cessna 150L

The airplane was destroyed when it impacted terrain at approximately 2015 Central time. Visual conditions prevailed for the positioning flight. The commercial pilot was fatally injured. According to friends and family, the pilot had scheduled a night flight lesson with a student to start at 2100, at a different airport. The pilot contacted a family member around 2000 and stated he was preparing to depart for the lesson.

June 12, 2009, Nampa, Idaho, Cessna 150

The commercial pilot was practicing touch-and-go landings in the tailwheel-equipped airplane. He reported that the wind direction changed to a quartering tailwind after the airplane touched down. The pilot stated that he then lost directional control of the airplane. During the accident sequence, the left main landing gear failed and the airplane ground-looped, resulting in substantial damage to the left wing and horizontal stabilizer.

December 5, 2008, Auxvasse, Mo., Cessna 150

After performing a couple 360-degrees turns and slow flight, the pilot attempted a power-off stall. To prepare for the power-off stall, the pilot retarded the throttle and applied carburetor heat. During the recovery of the power-off stall, the pilot “rapidly” applied engine power, closed the carburetor heat, and the engine lost power. The pilot attempted to restart the engine, however, the restart was unsuccessful. During the forced landing to a field, the pilot landed the airplane downwind, impacting crops and terrain. Examination revealed a buckled firewall.
