The airplane, operated by United Airlines, experienced a bird strike in its left engine while landing. Examination of the engine revealed all the fan blades were damaged, with three fan blades fractured. The left engine inlet cowl exhibited numerous impact marks on the inner barrel skin with two penetrations forward of the engine fan case.

June 13, 2011, Oswego, Ill., Boeing B-17G

At about 0947 Central time, the airplane experienced an in-flight fire and performed an emergency landing. One passenger sustained a minor injury. The three crew members and three other passengers were not injured. The airplane was substantially damaged as a result of the fire, which continued after it was on the ground. Visual conditions prevailed.

May 18, 2011, Point Mugu, Calif., Boeing 707-321B

The airplane collided with terrain during takeoff at 1727 Pacific time. The airline transport pilot captain, commercial pilot first officer and flight engineer sustained minor injuries. The converted aerial refueling tanker sustained substantial damage to its wings, fuselage and empennage from impact forces and a post-crash fire. Visual conditions prevailed. At about 20 feet agl, the number two (left inboard) engine throttle lever slammed back to the idle position. The airplane stopped climbing, and the captain adjusted the pitch slightly down to hold V2 speed.

August 1, 2009, Livermore Falls, Maine, Boeing A75N1 (Stearman)

At about 0845 Eastern time, the airplane was substantially damaged following a loss of engine power and forced landing. The private pilot and passenger were not injured. Visual conditions prevailed. An FAA inspector reported the engine lost power shortly after takeoff, and a forced landing was attempted at the airfields perimeter. During the landing, the airplane struck power lines before colliding with terrain. Structural damage to the wings resulted.

May 12, 2009, Houston, Texas, Boeing 737

The airplane, operated under FAR Part 121 as Southwest Airlines Flight 519, sustained minor damage and experienced a fire in the area of the right landing gear when three of the four main landing gear tires blew out during touchdown at 1946 Central time. The two flight crew, three cabin crew and 48 passengers evacuated on the runway, with two adult passengers suffering minor injuries. Visual meteorological conditions prevailed.

July 5, 2008, New York, N.Y., Boeing 767-300/Boeing 737-300

At 20:36 Eastern time a near-midair collision occurred at John F. Kennedy International Airport involving Cayman Airways flight 792 (CAY792), a Boeing 737-300, and Linea Aerea Nacional de Chile flight 533 (LAN533), a Boeing 767-300. According to initial reports, CAY792 was executing a missed approach to Runway 22L and conflicted with LAN533 as it was departing from Runway 13R. Controllers assigned diverging headings to both aircraft to resolve the conflict.

February 3, 2007, Palos Verdes, Calif., Waco UPF-7/Boeing A75N1(PT17)

The two airplanes collided in midair at about 1500 Pacific time, during an attempted formation flight of three airplanes. The Airline Transport pilot and sole occupant of the Waco sustained minor injuries, while the Private pilot and sole occupant of the Boeing was not injured. The Waco sustained substantial damage and the Boeing sustained minor damage. Visual conditions prevailed. According to interviews conducted by the NTSB, the Boeing was in a left echelon formation trailing another airplane when the Waco attempted to move into the left echelon position where the Boeing was already positioned.

May 20, 2006, Jennings, La. / Boeing A75

The airplane was substantially damaged at about 1830 Central time during a forced landing following a loss of engine power shortly after takeoff. The Commercial pilot and his passenger were not injured. Visual conditions prevailed. The pilot reported that while climbing through an altitude of 30 to 40 feet agl, the airplanes engine lost power. The pilot elected to turn toward an open field. Shortly before touchdown, as the pilot was attempting to avoid a fence, the airplane stalled and impacted the ground….

December 8, 2005, Chicago, Ill. / Boeing 737-700

One person on the ground died and 12 others suffered injuries when the Southwest Airlines flight slid off the runway at Chicago Midway Airport while landing. The airplane slid off the runway and went through a barrier fence and onto a roadway. There were 98 passengers onboard and five crew members on board. An emergency evacuation was accomplished….
