At about 21:20 Hawaiian standard time, a cargo-filled Boeing DC-10-30F sustained substantial damage to both elevators after responding to a TCAS alert five minutes after departure. No one was injured. The captain said the airplane departed runway 08R and he was turning right to a heading of 155 degrees and passing through 1,500 feet when air traffic control instructed him to turn left to 140 degrees for traffic. The traffic was at 10 oclock at 5,000 feet. The captain acknowledged the traffic in sight. The first officer advised the captain to shallow the climb to maintain separation. Sept. 2, Santa Barbara, Calif. / Boeing 737-322

At 14:06 PDT, United Airlines Boeing 737 encountered turbulence at FL270 near Santa Barbara, injuring one flight attendant seriously. Another flight attendant and 11 passengers sustained minor injuries. The flight had just leveled at FL270 and was crossing the San Marcus VOR when the turbulence was encountered. The aircraft was 10 miles in trail of the preceding aircraft on that route when the event occurred. No significant weather was forecast on the high altitude prognostic charts….

Nov. 18, Palm Springs, Calif. / Boeing 767

At about 09:00 PST, a Boeing 767-223 operated by American Airlines as Flight 160 suffered a rapid decompression during climb. The pilot made an emergency descent and landed without additional incident at the Palm Springs International Airport. Two of the 10 flight attendants and four of the 111 passengers reported minor injuries. The flight crew said the jet was climbing through 32,000 feet when they heard a rumbling sound followed by the illumination of the Center Duct Leak light on an overhead panel. April 2, Abilene, Texas / Boeing 757

At 22:32 CDT, a Boeing 757-2G7 operated by America West Airlines encountered turbulence while cruising at FL310 near Abilene. The airplane was undamaged, but three flight attendants in the aft galley sustained injuries (one serious and two minor), and one flight attendant was not injured. No one else on board was injured. The aircraft was en route from Orlando, Fla., to Las Vegas, Nev., when the captain noted a line of thunderstorms. At about 06:40 EST, a Boeing 737-3B7 operated by U.S. Airways had a flight attendant-initiated evacuation after pushback from the gate. No one was injured. U.S. Airways reported that the No. 2 engine had been bore scoped the night before and its ignition circuit breakers were pulled. The crew attempted to start the engine after pushback for the first flight of the day but was unable to. The maintenance crew told the crew to check the circuit breakers, which had not been reset. The crew started the engine and flames shot out the back. May 20, Nashville, Tenn. / Boeing 737-500

At about 20:10 CDT, a passenger on Southwest Airlines Flight 1857 sustained a fractured wrist while jumping from the airplane after arrival at Nashville. The Boeing 737-500 was about one hour into a ramp hold due to a weather-induced airport power outage and subsequent gate unavailability. Prior to jumping from the airplane, the passenger repeatedly left his seat, approached a flight attendant at one of the aft entry doors and told her he needed some air. She directed him to reseat himself. On the third such encounter the passenger grabbed the flight attendants hair, opened the slightly ajar door and jumped to the tarmac. At about 16:25 eastern time, a Boeing 757-28A operating as Avianca Airlines Flight 007 ingested one or more turkey buzzards during takeoff from Miami International Airports runway 27L. No humans were injured and no emergency evacuation was ordered following a precautionary landing to the same runway. The flight had just become airborne when it encountered a flock of at least 20 circling turkey buzzards. Jan. 5, Honolulu, Hawaii / Boeing 747

At about 16:20 Hawaiian time, a Boeing 747-200F operated by Evergreen International Airlines experienced smoke in the cockpit while en route from Honolulu to Pago Pago. The flight was about 2 hours out of Honolulu at flight level 340, but returned for an uneventful landing. Inspection found an incandescent lamp dimmer was still hot after two hours without electrical power. Neither of the two circuit breakers attached to the dimmer assembly were open.
