April 19, 2012, Gatlinburg, Tenn., Cessna 210 Centurion

At 1715 Eastern time, the airplane’s main landing gear collapsed during the landing roll. The airplane sustained substantial damage to the left horizontal stabilizer. The solo private pilot was not injured. Visual conditions prevailed. Upon lowering the landing gear at his destination, the pilot noticed the hydraulic unit stayed on and he did not have a green landing gear light. He confirmed the nose wheel was extended but both main landing gear were only partially extended. After attempting the emergency landing gear extension several times with negative results. After speaking with a mechanic by radio, the pilot attempted to refill the hydraulic reservoir.

April 20, 2012, Yorktown, N.Y., Beech A36 Bonanza

The airplane was substantially damaged during a forced landing following a loss of engine power during cruise flight. The accide nt occurred at 1626 Eastern time. The private pilot and passenger sustained minor injuries. Visual conditions prevailed, and an IFR flight plan was filed. The airplane was in cruise flight at 7000 feet when it lost engine power. The pilot determined the airplane would not reach an airport and selected a forced landing site. During the descent, the pilot performed remedial actions in accordance with the checklist, and switched the fuel selector from the right tank position to the left tank.

The airplane collided with terrain short of the runway at 1717 Pacific time. The commercial pilot sustained serious injuries; the two passengers were not injured. The airplane was substantially damaged. Visual conditions prevailed. The pilot was returning to the airport with an unsecured fuel cap when the airplane impacted terrain short of the runway.

At about 1015 Pacific time, the experimental airplane impacted the terrain. The private pilot and his flight instructor received fatal injuries. The airplane sustained substantial damage. Visual conditions prevailed. Initial radar data indicates the plane had been maneuvering at about 10,500 feet msl. The airplane then performed a level 360-deg.

April 27, 2012, Anderson, S.C., Cirrus SR22

The airplane was substantially damaged at about 1300 Eastern time when it impacted terrain shortly after takeoff. The private pilot/owner was fatally injured. Visual conditions prevailed. The pilot was practicing takeoffs and landings as his flight instructor watched. During the second circuit, the flight instructor observed the airplane’s landing light on final approach to the runway. The airplane appeared to land normally, touching down within the first 10 to 15 percent of the runway.

At about 1520 Eastern time, the airplane sustained a total loss of engine power during cruise flight and was landed off-airport. The airline transport pilot and two passengers were not injured. Visual conditions prevailed. According to the pilot, while in cruise flight, the engine began losing power until the engine ceased operation and he made a forced landing on a paved road. During the landing roll, the airplane nosed over and came to rest inverted, with substantial damage. Prior to departure, the airplane had been refueled.

The airplane sustained substantial damage following a loss of engine power and off-field landing at about 1032 Pacific time. The commercial pilot received minor injuries. Visual conditions prevailed. As the airplane climbed through about 2000 feet, the engine suddenly stopped developing power. The pilot reportedly switched to the auxiliary fuel tank shortly before the engine quit.

At 0958 Pacific time, the airplane lost engine power during flight and was landed on a road. The airplane sustained substantial damage. The private pilot and three passengers were not injured. Visual conditions prevailed. According to the pilot, during the flight he heard a change in engine power. He switched the fuel tanks and monitored the fuel flow. The fuel flow continued to decrease until the engine lost power.

March 2, 2012, Salisbury, N.C., Cessna LC42-550FG Columbia 350

At about 1340 Eastern time, the airplane was substantially damaged following a collision with water. The private pilot and passenger were fatally injured. Instrument conditions prevailed; no flight plan was filed. The pilot departed VFR and climbed to approximately 2000 feet. The airplane was observed on radar for approximately two minutes before descending to 1800 feet. Witnesses heard the airplane flying overhead but did not see it due to heavy fog. Shortly, they heard a loud splash in the lake and saw a large spray of water.
