August 2, 2012, Truckee, Calif., Piper PA-24-250 Comanche

At about 0814 Pacific time, the airplane sustained substantial damage upon impacting a hangar during takeoff. The solo commercial pilot was fatally injured. Visual conditions prevailed. Originally, the pilot and two passengers aborted a takeoff and landed uneventfully on the remaining runway. The pilot then had both passengers exit the airplane while the engine was still running and taxied back to Runway 19. On the second takeoff, the airplane lifted off normally, entered a nose-high attitude as if it was going to “stall,” then descended into an airplane hangar. The engine sounded normal and appeared to be producing power.

August 6, 2012, Chula Vista, Calif., Cessna 208 Caravan

While on short final, the airplane experienced a sudden sink. The pilot was unable to arrest the sink rate even after power was applied because of the lag time for the airplane’s turbine engine to spool up. The airplane landed hard, short of the runway. Examination revealed the left fuselage was dented and wrinkled, and the left brake assembly was missing.

August 7, 2012, Evansville, Ind., Cessna 177B Cardinal

At about 1116 Eastern time, the airplane was substantially damaged during recovery after a loss of control. The pilot and passenger were not injured. Instrument conditions prevailed; an IFR flight plan was on file. The pilot reported a loss of attitude reference and loss of control during cruise flight in IMC. He subsequently recovered the airplane after descending into VMC. Examination revealed both wings suffered wrinkled upper wing skins and wing structure deformation.

August 11, 2012, Effingham, S.C., Beech V35B Bonanza

At about 1310 Eastern time, the airplane was substantially damaged during a forced landing following a loss of engine power. The private pilot and passenger were not injured. Instrument conditions prevailed; an IFR flight plan had been filed for the flight. During cruise, ATC advised the pilot of an area of precipitation. The pilot requested a deviation; ATC advised the pilot to “turn left.” As he initiated the turn, the airplane encountered severe turbulence and the pilot’s primary flight display temporarily “went black.” The pilot used standby instrumentation for control and initiated an emergency descent. The airplane had exited turbulence and IMC at about 4000 feet msl when the pilot heard a “bang.”

August 15, 2012, Clifton Park, N.Y., Beech A36TC Turbo Bonanza

At 0727 Eastern time, the airplane was substantially damaged when it impacted trees and terrain during a forced landing. The airline transport pilot in the left seat was seriously injured; the commercial pilot in the right seat was fatally injured. Visual conditions prevailed. During initial climb, at 0725 and an altitude of 1100 feet msl, the pilot advised ATC, “eight delta romeo just lost our engine.”

August 20, 2012, McCall, Idaho, Cessna 182Q Skylane

The airplane sustained substantial damage at about 1050 Mountain time when it impacted trees in rising mountainous terrain during climb. The CFI and his commercial pilot-rated student both received minor injuries. Visual conditions prevailed. The CFI later stated they were flying up a mountain canyon when the terrain elevation increase exceeded the airplane’s climb performance. The airplane began impacting tree tops and subsequently impacted terrain. Density altitude was about 8600 feet.

August 22, 2012, Gulf of Mexico, Piper PA-28RT-201T Turbo Arrow IV

At about 1140 Eastern time, the airplane was substantially damaged during a forced landing into the Gulf of Mexico. The private pilot was not injured. Visual conditions prevailed. While en route to St. Petersburg, Fla., from Knoxville, Tenn., the pilot maneuvered the airplane around several thunderstorms before the engine began to run rough and lost partial power over the Gulf of Mexico.

At 1715 Eastern time, the airplane experienced a total loss of engine power on initial climb. The pilot attempted to return to the airport but the airplane collided with trees, sustaining substantial damage. The private pilot and one passenger received serious injuries. Visual conditions prevailed.
