At about 1842 Eastern time, the airplane was substantially damaged during a forced landing following multiple attempts to land at several airports in the vicinity. The solo instrument-rated private pilot was fatally injured. Night instrument conditions prevailed; an IFR flight plan was in effect. The flight originated in Sandersville, Ga.

January 16, 2013, Burlington, N.C., Pilatus PC-12/45

At about 0557 Eastern time, the airplane was substantially damaged when it impacted terrain shortly after takeoff. The airline transport pilot was fatally injured. Instrument conditions prevailed; an IFR flight plan was in effect. The pilot made initial ATC contact while climbing to the assigned altitude of 3000 feet msl. The pilot was told to reset his transponder and no further communications were received from the accident flight.

The airplane collided into a muddy field at about 1605 Pacific time following a total loss of engine power during initial climb. The flight instructor and passenger sustained minor injuries; the airplane sustained substantial damage. Visual conditions prevailed. The airplane had recently undergone an annual inspection; the accident flight was the first one since that maintenance was conducted. After takeoff, the instructor made the short flight to the destination and decided to perform several touch-and-go landings.

January 19, 2013, Mangum, Okla., Beech 95-B55 Baron

The airplane impacted terrain at about 1700 Central time, sustaining substantial damage. The commercial pilot and two passengers were fatally injured. Visual conditions prevailed. An annual inspection of the airplane had just been completed. The pilot started the airplane and did a taxi test down the runway, before stopping and picking up the two passengers.

January 22, 2013, Danbury, Conn., Cirrus Design Corp. SR20

At about 1925 Eastern time, the airplane was substantially damaged after its airframe parachute was deployed while on approach. The flight instructor, a private pilot and a passenger were not injured. Visual conditions prevailed. The airplane was on approach when it experienced a total loss of engine power and the pilot reported that the airplane was “out of fuel” to ATC. The pilot elected to deploy the CAPS and the airplane subsequently descended via parachute into trees, about three miles northeast of the airport.

January 23, 2013, McKinney, Texas, Piper PA-24-250 Comanche 250

The airplane was substantially damaged at 1200 Central time during a gear-up landing following an electrical system failure. The airline transport-rated pilot and passenger were not injured. Visual conditions prevailed. While approaching the destination, the airplane suffered a complete electrical failure. The pilot and passenger attempted to identify the cause of the electrical failure without success. Without electrical power, the landing gear required manual extension.

January 26, 2013, Atwater, Calif., Cessna 152

At about 1810 Pacific time, the airplane experienced a loss of engine power while on approach for landing. The pilot made a forced landing in an open field. During the landing rollout, the airplane struck a culvert and nosed over, coming to rest inverted. The solo student pilot was not injured. The airplane sustained substantial damage to its tail section and wings. Visual conditions prevailed.

January 2, 2013, Clear Lake, Iowa, Bell Helicopter 407

The helicopter impacted terrain at about 2057 Central time and was destroyed. The pilot and two medical crew members sustained fatal injuries. Night visual conditions prevailed. A witness about one mile south of the accident site observed the helicopter as it approached from the east. He noted that it appeared to slow and then turn to the north. When he looked again, the helicopter appeared to descend straight down. He subsequently went back into his house and called 911. He described the weather conditions as “misty,” with a light wind.
