The airplane impacted terrain at about 1700 Central time, sustaining substantial damage. The commercial pilot and two passengers were fatally injured. Visual conditions prevailed.
An annual inspection of the airplane had just been completed. The pilot started the airplane and did a taxi test down the runway, before stopping and picking up the two passengers. The flight then departed. Subsequently, witnesses reported the airplane “sputtered” before it nosed over and entered a spin, which appeared to flatten out before the airplane collided with the ground.
