September 23, North Las Vegas, Nev. / Piper Arrow and Piper Mirage

At 1255 Pacific time, a Piper PA-28R-200 and a Piper PA-46-350P collided at the crossing points of runways 7 and 12R at the North Las Vegas Airport. The Arrow was landing on runway 12R, and the Mirage had started its takeoff roll on runway 7. The pilot in the Arrow sustained a serious injury and the pilot in the Mirage sustained a minor injury. August 01, Oshkosh, WI / Vans RV-6T and Lancair 360

At about 1300 central time, a Vans RV-6T was struck by a Lancair 360 while the two amateur-built airplanes were taxiing after landing on runway 27 at Wittman Regional Airport. The RV-6 had landed and was exiting the runway when the Lancair landed and struck the RV-6s left wing. The accident occurred during the AirVenture fly-in, at which time controllers use the runway for several aircraft simultaneously….

August 22, Lake Elmo, Minn. / Cessna 172 and Piper Archer

At 1727 central time, a Cessna 172 and a Piper PA-28-181 collided in mid-air near Lake Elmo. The flight instructor in the Cessna was killed and the student pilot received serious injuries. The pilot and passenger in the Piper were killed. The Cessna had departed the Downtown St. Paul Airport on an instructional flight and was en route to Lake Elmo Airport. The Piper departed the South St. Paul Municipal Airport and was en route to Silver Bay, Minnesota….

August 16, West Palm Beach, Fla. / Piper Pawnee and Robinson R22

At about 1146 eastern time, a Piper PA-25-235 and a Robinson R22 Beta collided in flight at North Palm Beach County General Aviation Airport. The pilot of the Piper sustained serious injuries. The flight instructor and pilot-rated student of the Robinson helicopter both sustained minor injuries. The pilot of the glider being towed by the Pawnee released and landed uneventfully. The helicopter instructor said she announced on the common frequency that she would be making a 180-degree autorotation to runway 8L. June 17, Deerfield Beach, Fla. / Cessna 172 and Cessna 182

At about 1950 eastern time, a Cessna 172N and a Cessna 182Q collided while in cruise flight about 300 yard east of the beach at Deerfield Beach. The collision killed the pilot and two passengers in the 172 and the pilot and passenger in the 182. Both flights had originated at Fort Lauderdale Executive Airport. The 172 took off at about 1835 and the 182 at 1940. The airplanes were flying in opposite directions. Witnesses said the southbound airplane was a little lower than the northbound airplane, and just before collision the southbound airplane banked as if to take evasive action. April 05, Herald, Calif. / Piper Pawnee and Grumman Tiger

At about 1045 Pacific time, a Piper PA-36-375 collided with a Grumman American AA-5B while both airplanes were attempting to land at Bottimore Ranch Airport. Neither pilot was injured. The pilot of the Piper said he was landing when he heard the collision, then noticed the Grumman, which had just touched down on the same runway. He went around and diverted to Franklin Field Airport. The pilot of the Grumman said the aircraft went all over the place just after the main landing gear touched down. He said he did not know he had been hit by another airplane until he saw the Piper flying away from him….

April 08, Belle Glade, Fla. / Two Air Tractor 502s

At about 0745 eastern time, an Air Tractor 502B and an Air Tractor 502 collided on runway 09 at Belle Glade State Municipal Airport. The pilot of the 502B suffered minor injuries but the pilot of the 502 was killed. The 502B was started his takeoff roll but did not see the 502, which had landed and was taxiing off of the runway, until it was too late to avoid a collision. Neither airplane was equipped with a VHF radio for using the airports common traffic advisory frequency….

April 11, Redding, Calif. / Cessna 177RG and Piper Aztec

At about 1030 Pacific time, a Cessna 177RG collided with a parked Piper PA-23-250 prior to taxing for takeoff at Redding Municipal Airport. The pilot of the Cessna was not injured and the Piper was unoccupied. The pilot told investigators he was a CFI student and was preparing to practice flying from the right seat. During the starting sequence, the pilot inadvertently set the throttle at a quarter inch from full throttle instead of at a quarter inch from idle. When the engine started, the pilot was not able to stop the airplane. The right wing of the Cessna passed over the left wing of the Piper and struck the propeller….

January 11, Parker, Ariz. / Cessna 182 and Grumman Hellcat

At about 1600 mountain time, a Cessna 182H and a Grumman F6F-5 collided in midair about 1 mile east of Parker. The Cessna crashed and the pilot and passenger were killed. The Hellcat suffered a damaged left wing but landed safely without injury to the Hellcat pilot. The Parker Airport was hosting an air show and the Hellcat was participating in a display of warbirds. The Cessna was departing runway 19 with a left climbing turn from the downwind leg when it collided at midfield downwind with the Hellcat that was returning to the airport after an air-to-air photo shoot with another airplane. January 24, Denver, Colo. / Piper Cheyenne and Cessna 172

At 1722 mountain time, a Cessna 172P and a PA-31T collided in midair over Denver. The pilot and two passengers aboard the Cessna and the two pilots aboard the Piper were killed. Six people on the ground suffered minor injuries from falling debris. The Cessna had departed Centennial Airport about 22 minutes earlier, bound for Cheyenne, Wyo. The Piper departed Jeffco Airport about 12 minutes earlier, bound for Centennial. The Piper was flying VFR to the southeast in Class E airspace at 7,800 feet squawking a discrete transponder code and under radar contact, but the airplanes Mode C altitude readout was apparently missing.
