At approximately 1600 Central time, the airplane was substantially damaged when it impacted trees at the departure end of a runway during a landing attempt after a cross-country flight. Visual conditions prevailed. The private pilot and two passengers were seriously injured.

The airplane was substantially damaged at about 0940 Eastern time during a forced landing following an in-flight loss of engine power. The solo airline transport pilot was not injured. Day, visual conditions prevailed. About 20 minutes into the flight, the engine began to lose power. The pilot informed ATC and requested a vector to the closest airport.

The flight instructor and private pilot located in the left front seat were killed. The rear seat passenger received serious injuries. The airplane sustained substantial damage.

The airplane was substantially damaged when it impacted trees and terrain at 2115 Eastern time, shortly after takeoff. The two private pilots on board were fatally injured. Night visual conditions prevailed. Shortly before takeoff, the airplanes fuel tanks were topped off. The lineman subsequently reported the pilots did not sump the tanks but that the run-up and takeoff were normal.

The airplane was substantially damaged when it impacted terrain at about 1415 Eastern time. Visual conditions prevailed. The airline transport pilot was fatally injured. At about 1414, the FAAs Norfolk, Va., ATC facility recorded a transmission stating, “Mayday mayday mayday, Cessna five two six one four, about five south of Chesapeake Airport, over the big fields, have a flight control malfunction, looks like Im going down,” which was the only transmission recorded from the accident flight.

The pilot later stated the airplane experienced a total loss of engine power about of a mile from the approach end of Runway 9R at the Ft. Lauderdale/Hollywood International Airport (FLL). The private pilot and passenger received minor injuries. Visual conditions prevailed. The pilot and passenger reported taking off from a refueling stop at 1608.

The pilot reported flying into a canyon at 6100 feet msl with the intent to fly over it. During the maneuver, the airplane started to lose airspeed and altitude.

At about 2145 Eastern time, the airplane was substantially damaged when it impacted the ground in a residential area shortly after takeoff. Night visual conditions prevailed. The private pilot and one passenger were seriously injured; the other two passengers sustained minor injuries.

At 1058 Pacific time, a Boeing 737 operating as Southwest Airlines Flight 649 and a Cessna C172 were involved in a runway incursion. The airplanes came within 200 feet vertically and 10 feet laterally as the 737 was landing and the Cessna 172 was departing an intersecting runway after completing a touch and go. The event occurred during daylight visual conditions.
