June 3, 2005 in Jeanerette, La. / Beech 95-B55

At approximately 0730 Central time the airplane was substantially damaged when it impacted terrain following a loss of control during takeoff initial climb from the Le Maire Memorial Airport (2R1) near Jeanerette, La. The foreign-certificated Private pilot and passenger sustained fatal injuries. Visual conditions prevailed for the 411-nm flight destined for Point Lookout, Mo. A witness heard the airplanes engines pop several times during pre-takeoff taxi operations before clearing up. The airplane was destroyed when it impacted terrain while flying the VOR/GPS Runway 22 approach at approximately 2145 Mountain time. Night instrument conditions prevailed for the air ambulance repositioning flight. The ATP-rated pilot and two crew members sustained fatal injuries; one crew member sustained serious injuries. The cross-country flight originated at Steamboat Springs, Colorado, at approximately 2115 to pick up a patient. The piston twin was destroyed following a loss of control during its initial takeoff climb from the Harry P. Williams Memorial Airport (PTN), near Patterson, La., at 0954 Central time. The ATP-rated pilot and his dog were fatally injured. Visual conditions prevailed for the planned flight to the Houma-Terrebone Airport (HUM), near Houma, La. One witness observed the airplane climbing at a steep nose-high attitude with the gear and flaps retracted. The airplane was substantially damaged on impacting water while attempting to land at the Atlantic City Municipal Airport/Bader Field (AIY) in Atlantic City, N.J., at about 1215 Eastern time. The Private pilot and passenger were fatally injured. Visual conditions prevailed. A witness at the departure end of Runway 11 waiting to take off stated that he heard the pilot of the accident airplane report short final, followed by an announcement that he was going around because he had a problem. January 24, Rancho Cucamonga, Calif. / Beech Travel Air

At about 1626 Pacific time, a Beech 95 lost a propeller blade, which resulted in the right engine tearing from its mounts. The airplane then crashed, killing the pilot. Witnesses reported observing the right engine suspended below the right wing and the airplane entering a steep right bank turn until rolling upside down. The flight was a post-maintenance test flight. The airplanes annual had been signed off that day and the flight was the first after the engine and propeller were overhauled. Feb. 12, Henderson, Nev. / Beech 95 Travelair

At about 14:30 PST, a Beech 95 hit the ground after an inadvertent shutdown of one engine while landing at the Henderson airport. Neither the instructor nor the student pilot was injured. Investigators determined that the aircraft was on approach when the student pilot, who was flying, changed fuel tanks. He inadvertently shut off fuel to the right engine as he was looking for traffic. There was confusion in the cockpit and the appropriate emergency procedures were not executed. The flight instructor had assumed control when the aircraft struck the ground with a windmilling propeller….

Dec. 15, Mesquite, Texas / Beech BE-95-A55 Travelair

At approximately 14:00 CST, a Beech BE-95 was substantially damaged when it veered off the runway during a practice engine failure on takeoff maneuver at the Mesquite Municipal Airport. Neither occupant was injured. the instructor said that, after the student applied takeoff power, the instructor covered the mixture controls with a plastic checklist, and pulled the right engines mixture lever to idle cutoff. The student was not able to maintain directional control, and before the instructor could take the controls, the aircraft departed the runway on the right side onto soft, muddy ground.
