After overflying the destination runway, the crew made a steeper-than-normal approach to the 3880-foot-long runway due to terrain. According to the captain, a bump was felt near the threshold during the landing but it was not extreme. As the propellers were reversed, the airplane veered to the right. The crew corrected and the airplane tracked straight for about 2000 feet before veering sharply right, exiting the runway and spinning 180 degrees. Inspection of the runway threshold revealed several four-foot-tall piles of rocks and dirt.

Fuel flow was erratic, with a slight fuel leak from pump drains. Inspection revealed debris in servo screens. Teardown report showed drive couplings pitted, blades and liners scored, rotors worn. Debris in servo filter appeared to be pieces of the liner. Replaced both pumps. (The same engine-driven fuel pumps (p/n 200F5002) were installed new at engine overhaul. They were replaced at 372 hours and 389.7 hours. The pumps removed had 776.3 and 794.0 hours.

(By the way, I passed the same Cub today at very close range. This time he wasnt showing up at all on ADS-B). In your response, you asked readers to report other anomalies, so heres one from a week or so ago.

Toward the back of the magazine youre holding in your hand, in our Quick Turns department, theres a news item about the FAA formally transitioning to the ICAO-standard/international flight plan form for all domestic non-military operations. If youve been paying attention over the last few years, as we have, youll be happy to know a process that has seen several earlier deadlines come and go seems to have finally staggered across the finish line. As of August 27, the international flight plan form is the law of the land, so to speak.

Moreover, the FAAs Airman Certification Standards (ACS) for private and commercial certificates specify that pilots are to have knowledge of EM concepts for many maneuvers. They include emergency approach and landing, soft-field/rough-field landing, normal approach and landing, short-field landing, various types of water landings, power-off 180-degree accuracy approach and landing and go-around/rejected landing. The word knowledge implies pilots should have, at least, a basic understanding of EM concepts and be able to apply these concepts to tasks in the FAAs ACS.

Most of my articles for this journal focus on managing the risk of flying piston-powered general aviation aircraft, with examples of good and poor risk management. But risk management is at least equally critical in the world of operating airliners and turbine-powered transport category aircraft. Recent air carrier accidents provide illustration and lessons relevant to operating small general aviation aircraft, especially when designing and certifying them. In fact, and just as during flight operations, the job of managing risk in the design and certification is to identify, assess and mitigate that risk. These procedures apply even more objectively when using rigid design criteria, especially when they involve transport category aircraft.

After installation of a factory-rebuilt engine, a ground-run was performed. At all settings, the engine ran too rich, and adjustments had no effect. Aircraft was not airworthy. Removed fuel servo from engine, sent out for repair. Reinstalled fuel servo after repair, and engine performed satisfactorily. Returned aircraft to service.

In the decade-plus since the coming ADS-B mandate became a thing for U.S. aviation, those whose operations will be affected have fallen mainly into two camps: early adopters and those who put it off as long as possible. In this binary world, I freely admit to being something of an early adopter. And despite some cool-and less expensive-new gear on the market, Im happy with my choice to equip with ADS-B in 2016. Its likely those who have taken a wait-and-see attitude also are happy.

For example, a large flying club I was in a few years back had a pair of Cessna Cardinal RGs. They were getting a bit long in the tooth, but were roomy and relatively fast, and they were good cross-country airplanes. They also were configured basically the same, with two nav/comms but little else: no autopilot, for example, GPS or DME. After getting to know them both, I came to prefer the blue-and-white one over the orange version, since it was a bit younger and cleaner. Neither let me down, but one was sold to someone outside the club and, shortly thereafter, another pilot landed the remaining Cardinal RG gear-up.

While carrying out Canadian AD CF90-03-R2, large leaks were found in the area surrounding the muffler tail pipe area under the structure for supporting the heat muff shroud. No defects were noticed in a visual inspection of this area. No data tag was on the muffler, so its assumed it is an original unit. The operators logs dont show it ever being changed.
