At about 1500 Pacific time, the airplane collided with terrain during an off-airport, forced landing following a loss of engine power. The airline transport pilot and one passenger sustained minor injuries; the airplane was substantially damaged. Visual conditions prevailed.

May 6, 2009, Lantana, Fla., Beech K35 Bonanza

At about 1130 Eastern time, the airplane was substantially damaged during a forced landing after a loss of engine power during initial climb. The commercial pilot and the passenger/owner of the airplane were killed. Visual conditions prevailed. The airplane reached an altitude of 75 to 100 feet above the ground, when the engine “sputtered” or “coughed,” and then “quit.” The airplane struck two parked, unoccupied airplanes before impacting the ground and colliding with a tractor-trailer container.

At approximately 1030 Central time, the airplane was substantially damaged during a forced landing following the loss of power on both engines shortly after takeoff. The solo commercial pilot received minor injuries. Visual conditions prevailed.

April 11, 2009, San Diego, Calif., Piper PA-24-250

The airplane experienced an in-flight breakup during an uncontrolled descent at about 1054 Pacific time. The airplane was destroyed during the breakup sequence and post-impact ground fire. The private pilot and passenger were killed. Visual conditions prevailed; the flight was operating on an IFR clearance.

April 18, 2009, Dubre, Ky., Kitfox Experimental

At 1750 Central time, the airplane collided with the ground while maneuvering. Visual conditions prevailed. The airplane was substantially damaged and the private pilot was killed. A witness observed the airplane approaching his house and reported its fuselage was swaying from left to right. The airplane then made a steep left turn, with its left wing appearing perpendicular to the ground. The airplane reversed its course and leveled out. The wings were observed to wobble, then the airplane pitched down and collided with the ground in a nose-down attitude.

The airplane was substantially damaged at about 1030 Pacific time when it impacted mountainous terrain. Visual conditions prevailed. The commercial pilot and pilot-rated passenger were killed. The airplane came to rest in a near-vertical attitude on an approximate 40-degree slope. The leading edge of the left and right wings were crushed aft throughout their span. The forward area of the fuselage was crushed aft. All major structural components and flight controls of the airplane were located at the accident site.
