October 3, 2009, Whiteville, N.C., Piper PA-32R-300

At about 1930 Eastern time, the airplane was substantially damaged during a gear-up landing. The private pilot and passenger were not injured. Visual conditions prevailed. Earlier, the pilot reported a complete loss of electrical power, and that he was unable to lower the landing gear. Several attempts to lower the landing gear using the published emergency procedures also were unsuccessful.

October 4, 2009, Black Canyon City, Ariz., Piper PA-24-250 Comanche

The airplane was destroyed when it collided with terrain at about 0725 Mountain time. The airline transport pilot and his sole passenger were killed. Visual conditions prevailed. The U.S. Forest Service located the wreckage the next day, while fighting a wildfire near the accident site. The fire was initially reported the afternoon of the crash.

October 4, 2009, Louisville, Ky., Cessna 210

At about 1240 Eastern time, the airplane was substantially damaged during landing after the landing gear failed to completely extend. The flight instructor and commercial pilot were not injured. Visual conditions prevailed. During the flight, the landing gear and flaps (both of which were hydraulically actuated) were cycled several times without incident. Upon returning to base, the landing gear would not extend.

October 6, 2009, Aurora, Texas, Beech 100

At approximately 1450 Central time, the airplane was substantially damaged upon impact with terrain following a dual loss of engine power. The commercial pilot and three passengers were seriously injured. Visual conditions prevailed. According to a witness, the airplanes engines were making a “popping” sound during the descent.

October 6, 2009, Easton, Pa., Cessna 172S

The pilot stated he was performing a no-flap landing after completing a local flight. On short final, he realized he was low and initiated a go-around. With the application of power, the airplane rolled to the left and the left wheel struck an approximately eight-foot-high embankment located near the edge of the displaced threshold of the runway.

October 7, 2009, Renton, Wash., Cessna 172P

At the initiation of the takeoff roll following a touch-and-go landing, the student pilot added full power, but before she could rotate for takeoff, she experienced “…a hard time controlling the airplane on the ground.” She aborted her takeoff, but was unable to keep the airplane from departing the side of the runway, where it impacted a distance remaining sign.

October 8, 2009, Sims, N.C., Remos Aircraft G-3

The pilot reported flying above a reservoir at 500 feet approximately 30 minutes before sundown. As he completed a 180-degree turn, the sun was in his eyes. He put on a cap to block the sun from his eyes, and as he was adjusting the cap, the aircraft impacted the water.
