While on final approach, the pilot encountered a right crosswind, and then up- and downdrafts as she crossed over a cliff under the flight path. The pilot said the touchdown was uneventful but, as the airplane slowed during the landing roll, it encountered a right crosswind gust, and the right wing lifted.

December 6, 2009, Temple Bar, Ariz., Aerospatiale AS 355F1

At about 1030 Pacific time, the helicopter was substantially damaged when its left engine cowling door opened while airborne. The commercial pilot and six passengers were not injured. Visual conditions prevailed for the Part 135 air-tour flight. During cruise, the pilot heard a “thumping” noise followed by slight feedback within the cyclic control. The pilot contacted a second company helicopter to have them visually inspect the helicopter in-flight.

December 6, 2009, Dodge City, Kan., Beech V35 Bonanza

The airplane was substantially damaged after a hard landing at 1415 Central time following a loss of airspeed indication in IMC. The commercial pilot and one passenger were not injured. Instrument conditions prevailed at the time of the accident. The pilot later stated he encountered icing conditions and diverted. Before reaching the new destination, the airspeed indicator stopped working. While over the runway on landing approach, the airplane “fell straight down.”

The airplane was substantially damaged after impacting Lake Russell at about 2223 Eastern time. The non-instrument-rated private pilot and three passengers were fatally injured. Instrument conditions prevailed. After the flight took off and at about 2222, ATC issued an on-course clearance.

December 8, 2009, Fresno, Calif., Piper PA-38-112 Tomahawk

At about 1248 Pacific time, the airplane was substantially damaged during a forced landing following a loss of engine power during initial climb. The commercial pilot, sole occupant of the airplane, sustained minor injuries. Visual conditions prevailed. Witnesses observed the airplane take off and heard the engine sputtering.

December 10, 2009, Lexington, Ky., Beech 58 Baron

The airplane incurred minor damage while landing at about 1205 Eastern time. The airline transport pilot and commercial pilot were not injured. Visual conditions prevailed. During a go-around following a practice instrument approach, the crew retracted the landing gear. However, a controller advised the landing gear appeared to be in the extended position.
