January 6, 2012, Phoenix, Ariz., Piper PA-28-181 Archer II

At about 1548 Mountain time, the airplane was substantially damaged during landing when it veered off the runway. The student pilot was not injured. Visual conditions prevailed. Earlier, during a landing at a different airport, the airplane pulled to the right, but the pilot did not experience a runway excursion. After slowing, the pilot exited the runway, and the airplane taxied normally. The pilot shut down the engine and briefly examined the airplane, but did not observe any anomalies and departed on the next leg of a student cross-country flight.

January 11, 2012, Fitchburg, Mass., Cessna 150G

The airplane was substantially damaged at 0930 Eastern time during an impact with terrain following loss of engine power during initial climb. The flight instructor received minor injuries and the student pilot was seriously injured. Visual conditions prevailed. After two touch-and-go landings at a nearby airport, the engine began to run rough. The flight instructor took control, lowered the angle of attack and applied carburetor heat. The engine “immediately ran rougher” and the flight instructor pushed in the carburetor heat knob.

January 13, 2012, El Dorado, Kan., Cessna 150M

At about 2000 Central time, the airplane lost engine power during cruise flight. The solo private pilot was not injured but the airplane sustained substantial damaged during the forced landing. Night visual conditions prevailed. The pilot later stated he was at about 12,000 feet msl and wanted to see how far the airplane would glide, so he pulled back the mixture and throttled back. At about 8000 feet, the pilot added the mixture and throttle but the engine would not respond.

January 16, 2012, Philadelphia, Miss., Aerostar Aircraft Corp. PA-60-601P

At about 1242 Central time, the airplane experienced a loss of directional control during the initial takeoff and crashed in an open field. Visual conditions prevailed. The airplane sustained substantial damage due to impact and a post-crash fire. The solo airline transport pilot was fatally injured. One witness reported the accident airplane became airborne just before the intersection of the ramp and runway.

January 22, 2012, Quincy, Fla., Cessna 150G

At about 2015 Eastern time, the airplane was substantially damaged when it impacted trees and terrain shortly after taking off. The private pilot was fatally injured. Night instrument meteorological conditions prevailed; no flight plan had been filed. A pilot-rated witness reported the airplane landed after sunset, at about 1830, and taxied to the fuel pump. Shortly before the airplane departed, the witness noted fog was beginning to roll onto the airport.

January 25, 2012, Port Orange, Fla., Cessna 210E Centurion

The airplane experienced a loss of engine power while maneuvering in the traffic pattern at about 1515 Eastern time. The pilot made a forced landing short of the runway and the airplane was substantially damaged. Visual conditions prevailed; the private pilot received minor injuries. According to the pilot, it was the first flight after an engine installation and he planned to stay in the traffic pattern. Prior to departure he conducted an engine run-up and magneto check. After everything was checked, he took off and climbed to traffic-pattern altitude.

January 27, 2012, Ocala, Fla., Cessna 340A

At about 1227 Eastern time, the airplane was substantially damaged following a collision with terrain. The private pilot was fatally injured and one passenger received serious injuries. Visual conditions prevailed. The main wreckage was located about 0.65 nm from the runway. The cockpit and cabin were extensively burned from a post-impact fire. The landing gear handle was found in the retracted position. The position of the landing gear actuator linkage indicated an “in transit” position and was in close proximity to the up/retracted position.

The airplane crashed into Cape Cod Bay at about 1010 Eastern time. Instrument conditions were present and an IFR flight plan was filed for the instructional flight. The airplane sustained substantial damage. The flight instructor and private pilot were fatally injured.The crew was practicing instrument holding patterns as part of an instrument proficiency check. When ATC queried the crew about altitude fluctuations, the crew responded that there was smoke in the cabin.

The airplane was substantially damaged during a forced landing following loss of engine power at about 1455 Central time. The commercial pilot was not injured. Visual conditions prevailed. While descending through 2100 feet msl for his destination, the pilot configured the airplane for landing by reducing engine power to about 1500 rpm and setting the mixture to full rich. He did not activate carburetor heat.

January 21, 2012, North Vernon, Ind., Cessna 177 Cardinal

The airplane sustained substantial damage when it impacted wooded terrain at 2041 Eastern time. The private pilot and passenger sustained fatal injuries. Visual conditions prevailed and VFR flight following services were provided by ATC. Radar data showed the airplane in a descent from 7800 feet msl to 2800 feet msl before radar contact was lost. The airplane impacted wooded terrain adjacent to a residence. The homeowner reported he heard a low flying airplane, looked out the back window of his house and noticed a small fire in the woods behind his residence.
