The landing gear would not retract after takeoff, or extend when gear down was selected. The emergency gear extension handle was pulled, but it broke off in the pilot’s hand. The aircraft was landed gear-up. Troubleshooting revealed that the cable broke at the base of the handle.
Multiple critical component failures were reported, including a broken landing gear cable assembly leading to a gear-up landing and an emergency handle breakage.
Widespread issues were identified with Champion RHB32S spark plugs, showing numerous cases of cracked electrode insulators and high electrical resistance across various engine types.
Other significant defects included a Continental IO-360-ES Sky-Tec starter motor burning through its housing, an ECI cylinder with extensive cracking and a hole, and frequent failures of Goodyear inner tubes in a specific Cessna 182T fleet.
