The airplane experienced a loss of elevator control during landing at about 1545 Eastern time but was not damaged. The commercial pilot and flight instructor were not injured. Visual conditions prevailed.The two were practicing landings. The pilot conducted one landing and stated that the approach for the second landing was normal, but “slightly high.” During the landing flare, the pilot heard a “boom,” the airplane’s nose dropped and the airplane contacted the runway.

At 1738 Central time, the airplane was substantially damaged while landing. The flight instructor and pilot-receiving-instruction were not injured. Visual conditions prevailed. The flight instructor reported the airplane encountered a gust during the landing flare and drifted left of the runway centerline. He assumed control and increased engine power for a go-around, but the airplane impacted a ditch and taxiway.

February 4, 2012, Kalispell, Mon., Piper PA-28R-201T Arrow IV

The airplane experienced a partial loss of engine power at 1345 Mountain time, shortly after takeoff. The pilot attempted to land on a residential street but the airplane collided with parked vehicles and ended up inside a residence. The commercial pilot and his two passengers received minor injures; the airplane was substantially damaged. Visual conditions prevailed.

January 2, 2012, Del Norte, Colo., Piper PA-18-135 Super Cub

The private pilot was receiving instruction to become familiar with his newly acquired tailwheel-equipped airplane. After practicing some standard flight maneuvers at altitude, the pilot returned to the departure airport to practice full stop landings and takeoffs. On the fifth landing, the pilot made a three-point touchdown and rolled straight ahead. During the rollout, the airplane suddenly swerved to the right.

January 3, 2012, San Angelo, Texas., Piper PA-28-160 Cherokee 160

At approximately 1730 Central time, the airplane was substantially damaged during a forced landing. Visual conditions prevailed. The flight instructor and student pilot were not injured. The flight instructor reported they were performing touch-and-go landings in the traffic pattern. While on the downwind leg, the flight instructor changed fuel tanks in order to balance the fuel burn. Shortly thereafter, the student added power to adjust his approach path to the runway but the engine did not respond.

January 6, 2012, Phoenix, Ariz., Piper PA-28-181 Archer II

At about 1548 Mountain time, the airplane was substantially damaged during landing when it veered off the runway. The student pilot was not injured. Visual conditions prevailed. Earlier, during a landing at a different airport, the airplane pulled to the right, but the pilot did not experience a runway excursion. After slowing, the pilot exited the runway, and the airplane taxied normally. The pilot shut down the engine and briefly examined the airplane, but did not observe any anomalies and departed on the next leg of a student cross-country flight.

The airplane crashed into Cape Cod Bay at about 1010 Eastern time. Instrument conditions were present and an IFR flight plan was filed for the instructional flight. The airplane sustained substantial damage. The flight instructor and private pilot were fatally injured.The crew was practicing instrument holding patterns as part of an instrument proficiency check. When ATC queried the crew about altitude fluctuations, the crew responded that there was smoke in the cabin.

December 16, 2011, Merry Hill, N.C., Piper PA-38-112 Tomahawk

The airplane crashed during a forced landing following a loss of engine power at about 1330 Eastern time. The airplane received substantial damage and the non-certificated pilot/owner was seriously injured. Visual conditions prevailed. According to witnesses, the airplane was maneuvering at tree-top level towards an open field when the engine stopped producing power. The airplane then descended at a steep angle before it collided with terrain, nosed over and came to rest inverted.

December 19, 2011, Bryan, Texas, Piper PA-32-260 Cherokee Six

At about 2150 Central time, the airplane collided with terrain following an in-flight breakup. The instrument-rated private pilot and four passengers were fatally injured. Instrument conditions prevailed and an IFR flight plan had been filed. Review of ATC recordings revealed the pilot was diverting around an area of thunderstorms at the time of the accident. The pilot last reported that he was in “bad” weather and was going to try to get out of it.
