Antennas

During post-flight inspection, cracks were discovered on the ELTs rod antenna, approximately six inches from the base. The #2 GPS lost signal during an RNAV approach, generating a warning on the Garmin G1000. An operator noted it had experienced four-plus failures of the whip antenna installed for the Artex ME406 ELT. Flight experienced en route loss of GPS reception on all four devices, including a GTN 750/650 installation plus two portable WAAS GPS receivers. The GNS 430W indicated no satellite coverage while in flight. A portable GPS also indicated lost coverage.

February 14, 2012, Amory, Miss., Piper PA-32-260 Cherokee Six

The airplane was substantially damage during a forced landing into a wooded area, at about 0700 Central time, following a total loss of engine power. The commercial pilot was seriously injured. Visual conditions prevailed; an IFR flight plan was in effect. The pilot reported departing with six hours of fuel for the four-hour flight. While cruising at 6000 feet near Montgomery, Ala., the engine began to run rough. The pilot enriched the fuel mixture to alleviate the rough running engine, which resulted in higher fuel burn rate.

February 16, 2006, Lehigh Acres, Fla. / Piper PA-28-140 Cherokee

At about 2012 Eastern time, the aircraft collided with brush then nosed over during a forced landing to a road, following loss of engine power. Visual conditions prevailed. The airplane was substantially damaged; the Private pilot and one passenger sustained minor injuries. The pilot stated the fuel tanks were filled prior to takeoff. After takeoff and with the fuel selector positioned to the left tank, the engine experienced a loss of engine power. He reduced throttle, turned the fuel pump on and noted there was fuel pressure, then increased throttle, placed the mixture control to the full-rich position, moved the fuel selector to the right tank, and pumped the primer but was unable to rest…

May 13, 2005, Dover, Del. / Piper PA28-140

At about 2300 Eastern time, the airplane was substantially damaged following a collision with terrain during the initial climb from the Chandelle Estates Airport (0N4) in Dover, Del. The Commercial pilot and two passengers received minor injuries. Visual conditions prevailed for the flight, which was destined for Allentown, Penn. According to the pilot, he did not experience any problems with the airplane during the run-up, and that the engine lost power during the initial climb. The wreckage was located approximately mile off of the departure end of the runway….

March 17, Goldwaite, Tex. / Piper Cherokee

At approximately 1600 central time, a Piper PA-28 was substantially damaged following a loss of engine power while landing on a private grass strip. The sole occupant of the airplane, was not injured. According to the pilot, he was on the downwind leg when the engine began to run rough. The pilot advanced the throttle, however the engine continued to run rough. The aircraft landed in heavy brush, approximately 150 yards short of the runway. An FAA inspector who inspected the accident site reported that the fuel pump circuit breaker was tripped….

November 21, Sweetwater, Texas / Piper Cherokee 235

At approximately 0800 central time, a Piper PA-28-235 sustained substantial damage during a forced landing following a loss of engine power while in cruise flight. The pilot and one passenger were not injured. The flight was en route from Hobbs, N.M. to Abilene when the engine lost total power. The pilot attempted to reach Avenger Field Airport near Sweetwater for the forced landing, but headwinds prevented him from reaching the airport. The airplane collided with terrain about 400 feet short of the runway, then struck trees, a fence, and two poles before coming to rest on the runway….

August 06, Coatesville, Pa. / Piper Cherokee

At about 1035 eastern time, a Piper PA-28-235 crashed into trees after losing engine power while on approach to Chester County Airport. The pilot was seriously injured. The pilot had flown from the Chicago area, destined for Coatesville, and was about one mile out from runway 11 when the engine lost power. The pilot attempted to make a downwind landing to runway 29 but crashed a half-mile short. An inspector determined the airplane had run out of fuel….
