Antennas

During post-flight inspection, cracks were discovered on the ELTs rod antenna, approximately six inches from the base. The #2 GPS lost signal during an RNAV approach, generating a warning on the Garmin G1000. An operator noted it had experienced four-plus failures of the whip antenna installed for the Artex ME406 ELT. Flight experienced en route loss of GPS reception on all four devices, including a GTN 750/650 installation plus two portable WAAS GPS receivers. The GNS 430W indicated no satellite coverage while in flight. A portable GPS also indicated lost coverage.

NTSB Reports: January 2016

A pilot reported hearing a Mayday call from another airplane, stating that the engine had failed and he was attempting to land on the east side of a canal. No further communications were received from the accident airplanes pilot.

April 6, 2013, Circle, Mon., Cessna 182C Skylane

At about 1045 Mountain time, the airplane collided with terrain. The private pilot was fatally injured; the airplane was destroyed. The airplane was found after family members reported it overdue. The wreckage was found at about 1830. There were no witnesses and no reports of a distress call. Initial examination revealed damage and ground scars consistent with a high-energy vertical impact.

August 20, 2012, McCall, Idaho, Cessna 182Q Skylane

The airplane sustained substantial damage at about 1050 Mountain time when it impacted trees in rising mountainous terrain during climb. The CFI and his commercial pilot-rated student both received minor injuries. Visual conditions prevailed. The CFI later stated they were flying up a mountain canyon when the terrain elevation increase exceeded the airplane’s climb performance. The airplane began impacting tree tops and subsequently impacted terrain. Density altitude was about 8600 feet.

July 5, 2012, Flagstaff, Ariz., Cessna T182T Turbo Skylane

The pilot encountered a downdraft during his approach. The airplane touched down hard and bounced. After bouncing down the runway twice, the pilot decided to abort the landing and maneuvered the airplane back into the traffic pattern for the same runway. He landed without incident. Upon exiting the airplane, he noted the propeller had struck the runway on his first landing attempt. The airplane sustained damage to the firewall. The pilot reported that there were no pre-impact mechanical malfunctions or failures that would have precluded normal operation.

February 9, 2012, Lebanon, N.H., Cessna T182T Turbo Skylane

At 1345 Eastern time, the airplane was substantially damaged when it impacted terrain while returning to land shortly after takeoff. The private pilot was fatally injured. Visual conditions prevailed. A flight instructor preparing for a flight heard the accident pilot on the tower frequency informing the controller that he “had to come back.” He then observed the airplane flying northeast to southwest over the field, and heard the tower controller clear the airplane to land on any runway. The accident airplane continued toward the approach end of Runway 36.
