At about 1116 Eastern time, the airplane was substantially damaged during recovery after a loss of control. The pilot and passenger were not injured. Instrument conditions prevailed; an IFR flight plan was on file. The pilot reported a loss of attitude reference and loss of control during cruise flight in IMC. He subsequently recovered the airplane after descending into VMC. Examination revealed both wings suffered wrinkled upper wing skins and wing structure deformation.

January 21, 2012, North Vernon, Ind., Cessna 177 Cardinal

The airplane sustained substantial damage when it impacted wooded terrain at 2041 Eastern time. The private pilot and passenger sustained fatal injuries. Visual conditions prevailed and VFR flight following services were provided by ATC. Radar data showed the airplane in a descent from 7800 feet msl to 2800 feet msl before radar contact was lost. The airplane impacted wooded terrain adjacent to a residence. The homeowner reported he heard a low flying airplane, looked out the back window of his house and noticed a small fire in the woods behind his residence.

The airplane was substantially damaged at about 1005 Mountain time during a forced landing after a partial loss of engine power. The solo airline transport pilot was not injured. Visual conditions prevailed. The pilot reported a satisfactory preflight inspection and run-up. After taking off from the 6764-foot-elevation airport and at about 500 feet agl, he detected a vibration in the engine and observed the manifold pressure drop from 25 inches to 20 inches and the rpm drop from 2500 to 2300. The pilot turned on the fuel boost pump, but no change in engine performance was observed.

The airplane sustained substantial damage during a forced landing onto a road at about 1730 Pacific time. The commercial pilot and the pilot-rated passenger were not injured. Visual conditions prevailed. The pilot reported visually checking fuel quantity and determining he had enough fuel to make the cross-country flight and return.

The airplane was substantially damaged during an aborted takeoff at about 1415 Eastern time. The commercial pilot and passenger were not injured. Visual conditions prevailed. After flying in for lunch, the pilot and his passenger attempted to depart. The airplane accelerated and lifted off the runway normally. However, the elevator control was “frozen” and could not be moved.

The airplane sustained a loss of engine power and was ditched at about 1205 Eastern time. Visual conditions prevailed. The airplane sustained substantial damage; the private pilot and passenger were not injured. While preparing to land, the pilot moved the propeller control to the low pitch position and applied carburetor heat, but the engine quit. He immediately removed carburetor heat engine power was not restored.
