August 31, 2012, Rye, Colo., Piper PA-12 Super Cruiser

At about 0920 Mountain time, the airplane was substantially damaged after impacting mountainous terrain. The private pilot and passenger were fatally injured. Visual conditions prevailed. According to law enforcement witnesses, the pilot and passenger arrived at their planned surveillance location and established radio contact with personnel on the ground. These personnel and other witnesses observed multiple passes of the airplane at about 500 to 1000 feet agl, followed by maneuvering of the airplane out of their sight.

March 5, Salem, Ore. / Piper PA-12 Super Cruiser

At approximately 16:30 PST, a Piper PA-12 was substantially damaged when the aircraft struck up-sloping terrain just short of the runway threshold at the pilots personal landing strip near Salem. The pilot suffered minor injuries. May 19, Wasilla, Alaska / Piper Super Cruiser

At about 12:00 Alaska daylight time, a tundra tire-equipped Piper PA-12 sustained substantial damage during takeoff from a private airstrip. The pilot, who held a student pilot certificate, and passenger received minor injuries, while a second passenger was not injured. The pilot refused to talk to an FAA inspector and refused to provide his name or discuss the details of the accident with the NTSB investigator-in-charge. The accident was reported by the president of the homeowners association where the airstrip is located, who notified the Alaska State Troopers office and talked to the NTSB.
