At 1646 Central standard time, the airplane was substantially damaged during a runway excursion after landing. The private pilot was not injured. Visual conditions prevailed. The pilot later stated he applied the brakes to slow the airplane for a left turn off the runway after landing. Instead, the airplane began a turn to the right, which could not be corrected with left brake.

The airplane was substantially damaged after impacting Lake Russell at about 2223 Eastern time. The non-instrument-rated private pilot and three passengers were fatally injured. Instrument conditions prevailed. After the flight took off and at about 2222, ATC issued an on-course clearance.

November 1, 2009, Grangeville, Idaho, Piper PA-18-150

The pilot overflew the 800-foot long remote airstrip and observed the windsock hanging limp. About the time he reached the approach end of the uphill-sloping runway, the airplane encountered a strong tailwind gust, which pushed it “…too far up the strip to land.” The pilot executed a go-around, but his decision was “too late,” and he was unable to out-climb terrain.

November 14, 2009, Dennisville, N.J., Piper PA-28R-200

The airplane was substantially damaged when it impacted terrain at about 1050 Eastern time, some ten minutes after takeoff. The non-instrument-rated private pilot/owner, and his 12-year-old son were fatally injured. Instrument conditions prevailed; no flight plan was filed. According to several witnesses, the pilot originally planned to depart on November 12, but poor weather caused him to delay his departure. The following day, the pilot again decided not to depart due to the weather.

The airplane experienced a partial loss of engine power in cruise flight and the pilot made an emergency landing on a highway at 1553 Mountain time. The private pilot and passenger were not injured. The airplane sustained structural damage to the fuselage after departing the highway and impacting cacti prior to coming to rest upright. Visual conditions prevailed.

At about 1930 Eastern time, the airplane was substantially damaged during a gear-up landing. The private pilot and passenger were not injured. Visual conditions prevailed. Earlier, the pilot reported a complete loss of electrical power, and that he was unable to lower the landing gear. Several attempts to lower the landing gear using the published emergency procedures also were unsuccessful.
