At about 1101 Eastern time, the helicopter was substantially damaged during power-line installtion operations. Visual conditions prevailed. The commercial pilot sustained minor injuries and an additional crewmember sustained serious injuries. Preliminary examination revealed a rope was wrapped around the main rotor mast.

February 4, 2007, Miami, Fla., McDonnell Douglas DC-8-71F

At about 2255 Eastern time the Colombian-registered airplane, operating as an international cargo flight sustained minor damage when its right main landing gear collapse during the landing roll. Instrument conditions prevailed. The three crewmembers aboard were not injured. According to the Captain, after making a normal approach and touchdown, and with the airplane decelerating through about 100-120 knots, it began to lean and veer to the right. The crew stopped the airplane on the runway and evacuated.

October 12, Denver, Colo. / McDonnell Douglas DC-10

At 0718 mountain time, a McDonnell Douglass DC-10-10, operated by Federal Express, suffered damaged tire and brake assemblies during a rejected takeoff at Denver International Airport, necessitating closure of one runway for over 17 hours. The captain, first officer, flight engineer, and four non-revenue passengers were not injured. During acceleration for takeoff on runway 16R, the crew received a warning horn and initiated a rejection of the takeoff. Tower personnel alerted the crew to smoke and fire coming from the left gear and alerted firefighters. The crew completed their checklists and exited the aircraft via an evacuation slide. June 1, Little Rock, Ark. / McDonnell Douglas MD-82

American Airlines Flight 1420 crashed after landing at Little Rock. There were thunderstorms and heavy rain in the area at the time of the accident. The airplane departed the end of runway, went down an embankment and struck approach light structures. There was a crew of six and 139 passengers on board the airplane. Eleven people were killed in the accident, including the pilot. One person was seriously injured and four suffered minor injuries….

July 02, Houston, Texas / McDonnell Douglas MD-88

At 0922 central time, a McDonnell Douglas MD-88 operated by Delta Air Lines collided with parked construction trucks at Houston Hobby Airport. There were no injuries. The flight had been cleared to cross runway 22 and taxi to the gate. The pilot taxied on taxiway Zulu, which is parallel to taxiway Yankee, which was open. Taxiway Zulu was closed due to construction; however, the construction was not posted on the ATIS, NOTAM, or dispatchers release from the company. Ground control did not mention that taxiway Zulu was closed….

At about 11:38 CDT, a TWA DC-9 had its landing gear separate on rollout at Nashville International Airport. Three passengers were treated for minor injuries. The First Officer said he turned final and his sink rate was 600 to 800 fpm. The airplane touched down hard on the right main landing gear, which surprised him and the PIC. The PIC immediately got on the flight controls with him as the airplane bounced. On the second impact he felt a vibration, and it became evident that they had a directional control problem. At 19:03 CST, a Douglas DC-9-31 operating as TAESA Flight 725 crashed just after takeoff from UPN. All 18 aboard were killed. Witnesses said the airplane assumed a higher than normal climb attitude as soon as it departed from runway 20. The airplane impacted the ground in a nose low attitude on a heading of 110 degrees in an avocado grove located on the east side of the departure course, 3.3 miles south of the airport….

At about 16:20 PST, Alaska Airlines Flight 261, a Boeing (McDonnell Douglas) MD-83 on a scheduled international passenger flight from Puerto Vallarta, Mexico, to San Francisco, Calif., crashed into the Pacific Ocean near Point Mugu. The two pilots, three flight attendants, 80 adult and child passengers, and three infant passengers were killed. The U.S. Coast Guard conducted a search and rescue mission for 44 hours….

At 19:51 PST a Douglas DC-8-71F operated as a scheduled cargo flight by Emery Worldwide Airlines crashed shortly after takeoff from Mather Field. The three crew members were killed. The flight had originated in Reno, Nev., and arrived in Rancho Cordova late. The crew still made a 58-minute turnaround. As it departed runway 22l, it rotated about halfway down the runway, banked to the left at low altitude and porpoised. A witness said he saw the airplane impact the ground in a left-wing-low, nose-low attitude, with the engines at a high power setting. At about 06:35 eastern time, a Douglas DC-3 nosed over during landing at Douglas International Airport. The pilot said he made a normal IFR approach to runway 5 and had a gear down-and-locked indication. On touchdown, the airplane yawed right and nosed over. Examination of the airplane revealed that the right landing gear had collapsed….
