The airplane collided with the water at about 1108 Central time. Visual conditions prevailed for the IFR flight from Louisiana to Florida. The solo private pilot was fatally injured and the airplane received substantial damage. Approximately 20 minutes after levelling at FL 270, the airplane began to deviate from its altitude and route. Military fighter aircraft intercepted the airplane and reported the airplane was circling in a left-hand turn and the windows were partially frosted over.

December 9, 2011, Pampa, Texas, Cessna 421B

The pilot was flying a GPS approach in instrument conditions with icing present. The pilot later stated the airplane broke out of the weather about 300 feet agl and 300 feet right of the runway, with as much as ¼-inch of ice on the wings. The pilot corrected but overshot the runway, landing hard in the grass. The airplane traveled about 1000 feet before re-entering the runway. Post-accident examination revealed the right wing’s rear spar was bent.

At about 1740 Central time, the airplane was substantially damaged when it impacted terrain while on approach. The private pilot and six passengers were fatally injured. Visual conditions prevailed; an IFR flight plan was in effect. The airplane was in cruise flight at FL210 when the pilot declared an emergency due to a rough running engine. He diverted, subsequently confirming he had shut down the right engine and reported the airport in sight.

At about 1440 Pacific time, the airplane experienced an uncommanded retraction of the right flap while on the downwind leg for landing. The commercial pilot and two medical flight crewmembers were uninjured; the airplane was undamaged. Visual conditions prevailed.About the time the flaps reached their maximum limit, the pilot heard a popping sound, and the airplane simultaneously rolled about 80 degrees to the right. He counteracted with full left aileron, retracted the flaps and applied full engine power.

The emergency medical services flight was operating on an IFR flight plan under FAR Part 135. The airline transport pilot, two flight nurses, the patient and a passenger were fatally injured.

At approximately 1755 Central time, the airplane sustained minor damage during an in-flight separation of the elevator trim cable following runaway activation of the electric trim system. The airplane was in visual conditions at approximately 6000 feet msl on an autopilot descent from an en route cruise at 19,000 feet. The solo pilot was not injured.

The airplane was substantially damaged when it impacted a home at about 1115 Eastern time, following a loss of engine power shortly after takeoff. The solo commercial pilot was killed. Visual conditions prevailed. Prior to the accident flight, the pilot flew the airplane from Costa Rica to Florida, where it sat for two days. The pilot was ferrying the airplane to a new owner.
