The helicopter impacted terrain at about 2057 Central time and was destroyed. The pilot and two medical crew members sustained fatal injuries. Night visual conditions prevailed. A witness about one mile south of the accident site observed the helicopter as it approached from the east. He noted that it appeared to slow and then turn to the north. When he looked again, the helicopter appeared to descend straight down. He subsequently went back into his house and called 911. He described the weather conditions as “misty,” with a light wind.

At about 955 Eastern time, the helicopter experienced an in-flight bird strike, main rotor vibration and a hard landing. The private-rated pilot, commercial-rated copilot, and three passengers received minor injuries. The helicopter sustained substantial damage. Visual conditions prevailed.

The helicopter crashed into the East River during takeoff from the East 34th Street Heliport at 1525 Eastern time. Visual conditions prevailed. Two passengers sustained serious injuries and one passenger was killed. The commercial pilot and one passenger were not injured. The helicopter sustained substantial damage.

At 1756 Central time, the helicopter was force-landed during a post-maintenance test flight. Day visual conditions prevailed. The solo airline transport pilot was not injured. The helicopter sustained substantial damage to the tailboom section just aft of the cabin.According to the pilot, the engine governor was replaced earlier in the day and the test flight was to confirm proper inflight settings.

At about 1150 Eastern time, the helicopters pilot performed an emergency landing next to a roadway after impacting electrical wires. Visual conditions prevailed.
