While arriving at the destination airport, the pilot reported lowering the landing gear and flaps while on the downwind leg to the runway. He added that as he lined up for the runway, the airplane drifted to the right, indicating a strong wind from the west. After the airplane crossed the beginning of the runway, the drift increased and the pilot tried to correct back to the runway centerline.

At 1050 Eastern time, the airplane sustained substantial damage during takeoff. The student pilot was seriously injured. Visual conditions prevailed for the planned solo cross-country flight. The airplane was departing Runway 6 when it veered to the left and went off the runway. The airplane appeared to have cartwheeled before it came to rest upright about 200 feet from the runway.

The airplane was substantially damaged at about 1045 Eastern time when it impacted an airport perimeter fence following a loss of engine power on takeoff. The commercial pilot and the pilot-rated passenger were not injured. Visual conditions prevailed. Prior to departing a nearby airport, the pilot serviced the airplane to have about 20 gallons of fuel aboard. After picking up his passenger, the pilot taxied the airplane and performed a brief run-up. The pilot initiated the takeoff roll normally and observed normal instrument indications.

January 1, 2012, Willimantic, Conn., Seastar XP Experimental

At about 1211 Eastern time, the airplane was substantially damaged during a forced water landing after a partial loss of power during initial climb. The private pilot/owner was not injured. Visual conditions prevailed. The pilot was executing a series of touch-and-go landings. After touching down, adding power and initiating climb, the airplane’s engine lost partial power, dropping to approximately 2000 rpm. The pilot elected to do a water landing on the Willimantic Reservoir, which was close to the end of the runway, but had too much speed. During a series of S-turns, the left lower wing’s float struck the water and separated from the airplane.

At about 1545 Eastern time, the airplane was substantially damaged during a ditching in the Chesapeake Bay following loss of engine power. Both the airline transport pilot and the passenger successfully exited the airplane. After the ditching, the pilot and passenger attempted to swim to Smith Island; The pilot survived with minor injuries. The passenger was unable to complete the swim and died. Visual conditions prevailed.

The helicopter was substantially damaged during an autorotation to the ground at 1221 Mountain time following a loss of engine power. The commercial pilot and his two passengers were seriously injured. Visual conditions prevailed for the local tour flight, which had originated approximately 20 minutes before the accident.
