The airplane was destroyed when it departed from cruise flight at 0920 Eastern time and impacted terrain. The Commercial pilot and three passengers received fatal injuries. The flight departed Sheboygan, Wis., at 0738 and was en route to Lebanon, Ohio. Ground fog was present in the area of the accident site; however, the airplane was in visual conditions on an IFR flight plan at its cruise altitude. Radar track data indicate that, at 0919:42, the airplane was on a heading of 135 degrees at 7000 feet msl. At 1419:47, radar data indicate the airplane at 6900 feet msl, and descending. The certified Flight Instructor, pilot-rated passenger and non-rated passenger received fatal injuries and the airplane was destroyed at 1013 Eastern time after colliding with a power line, trees and the ground while on an ILS approach to Runway 5 at the Donaldson Center (S.C.) Airport. At 1327 Mountain time, the airplane was destroyed and its solo ATP-rated pilot fatally injured upon impact with terrain 500 feet west of Runway 20 at the Santa Fe (N.M.) Municipal Airport. Visual conditions prevailed for the aerobatic flight, which was the second performance in the 2004 Santa Fe Air Show. An FAA inspector witnessing the airshow later reported that the airplane was high, at approximately 1500 feet AGL, when it entered an inverted spin. At about 1400 local time, the twin-engine floatplane was substantially damaged after colliding with trees while on approach to Massapoag Lake in Sharon, Mass. The solo Private pilot was fatally injured. According to the FAA, the pilot diverted to Massapoag Lake for unknown reasons. While on approach, the airplane struck trees and impacted a swamp area. The Experimental airplane was substantially damaged during an attempted landing on a private airfield at about 1745 Eastern time; the Private pilot was seriously injured. According to the pilot, he performed a touch-and-go landing at his private airfield and, during the climbout, observed fuel venting from the right wing. He then switched the fuel tank selector from the left tank to the right tank. He then flew the downwind leg of the traffic pattern. Shortly before noon eastern time, the ATP-rated pilot attempted to land the Diamond DA-40 at a private field. Neither the pilot nor his passenger were injured. The pilot said he circled the field twice before setting up for a full-flap landing to the south. Despite flying as close as possible to trees on final and performing a slip to lose altitude, the airplane landed half-way down the 2,500 foot-long turf runway. At 0805 mountain time, a Fairchild, SA227-DC operated by Key Lime Air as Flight 375 slid off of the runway during the landing roll at Garfield County Regional Airport. The captain and first-officer were not injured. The pilot said he flew the instrument approach at a higher speed than usual and, as a result, the airplane touched down approximately 2,500 feet beyond the approach end of the runway 26, which was wet with rain. Despite the pilots attempt to stop the airplane with brakes and full reverse on the engines, the airplane began to slide sideways, and went off of the departure end of the runway, where it was substantially damaged….

At 0953 Pacific time, an amateur-built Thorpe T-18 rolled over and landed inverted at Compton/Woodley Airport, killing the pilot. The airplane had been flying for about 40 minutes, making several takeoffs and landings. The pilot of a Robinson helicopter, also in the pattern, said the Thorpe pilot was announcing his intentions on the common frequency. Meanwhile, an EC-135 helicopter called, reporting two miles east of the airport for a straight-in approach to runway 25R.
