Pilot error and judgment issues, such as inadvertently retracting flaps during takeoff, ignoring pre-flight warnings, or difficulties during go-around attempts, contributed to a number of accidents.
A significant portion of these accidents, particularly those involving major engine or control system malfunctions, tragically resulted in fatalities.
At about 1218 Central time, the airplane was substantially damaged when its pilot deployed the airframe parachute. The pilot and one passenger were not injured while a second passenger sustained minor injuries.
