The airplane impacted terrain at about 1700 Central time, sustaining substantial damage. The commercial pilot and two passengers were fatally injured. Visual conditions prevailed. An annual inspection of the airplane had just been completed. The pilot started the airplane and did a taxi test down the runway, before stopping and picking up the two passengers.

The pilot had flown into the destination airport many times before and was familiar with landing there. While completing the before-landing checklist, the pilot became distracted by passenger conversation. The pilot never heard the gear warning horn and before he realized that the gear was still up, he impacted the runway. The airplane sustained substantial damage to the fuselage during the wheels-up landing. No one aboard was injured.

The pilot lost control shortly after takeoff from a private airstrip and crashed into a wooded area at about 1600 Central time. Visual conditions prevailed. The airplane sustained substantial damage; the solo private pilot was fatally injured.

November 17, 2011, Ulysses, Penn., Beech 95-C55 Baron

The airplane was substantially damaged at about 1619 Eastern time during a forced landing following a total loss of engine power. The airline transport pilot and three passengers were seriously injured. Visual conditions prevailed. The airplane was in cruise flight at 8000 feet msl when the pilot requested a descent to 6000 feet due to turbulence and rime icing. The pilot then switched both engines from auxiliary to main fuel tanks. Shortly, the left engine began to surge.

The airplane was substantially damaged at about 0830 Central time when it impacted terrain shortly after takeoff. The solo private pilot was fatally injured. Visual conditions prevailed. According to a witness, the airplane was an estimated 300 to 500 feet in the air when its engines sounded like they were cutting out.

The airplane was substantially damaged when it crashed about 1845 Central time during an instrument approach. Night instrument conditions prevailed and an IFR flight plan was filed. The private pilot was killed. The pilot was cleared for the ILS Runway 24 approach at BHM, which he acknowledged. The airplane initially intercepted the localizer for the approach, but did not intercept the glideslope.

At about 0920 Eastern time, the descended into a house. Visual conditions prevailed. The airplane was destroyed by impact and a post-crash fire. The commercial pilot and airline transport pilot safety pilot were killed. The house was destroyed but the occupant of the house was not injured.

The airplane incurred substantial damage at about 0915 Eastern time during a right engine start. The pilot was not injured. Visual conditions prevailed. After both wings were filled with fuel to capacity, the flight departed. At approximately 700 feet agl, the right-wing fuel cap released from its opening but remained attached by a chain. Fuel was observed coming from the opening.
