High blood presure and Diabetes

I have high blood pressure and diabetes which I take meds for and is under control. What must I do to pass my medical again I did hold a class II medical 4 years ago. Must I tell my flight doctor and will there be any limitations? What about a DWI I had 3 years ago must I report that. If you complete the re-edcuation it was suppose to not go on your record.
