While I haven't kept a "lessons learned" log, per se, I always give myself a post-flight briefing and itemize in my mind what I did right and what I did wrong, and especially what led to doing something wrong, such as a distraction, or not feeling quite well, or whatever. It's a good practice, because of course none of us, no matter how experienced or current, are perfect pilots. I frequently say that a certificate is a license to learn--and that flying is a constant learning experience. When any pilot gets to the point when he or she thinks that he or she knows all there is to know about flying, it's time to stop flying altogether.
